Recovery and stabilization requires more than a hot meal and a bed. Our Life Coaches work with clients to help them achieve reintegration into a larger and healthier culture. Our goal is to obtain these objectives through four basic phases:
Crisis Intervention & Short-Term Stabilization: This phase includes helping individuals access emergency shelter services and/or short-term transitional housing and address crisis needs.
Screening, Intake and Needs Assessment: The "needs assessment" results in an action plan for clients, which includes short- and long-term goals and objectives with concrete action steps toward attaining permanent housing and long-term stabilization.
Plan Management: After the completion of screening and assessment, the next phase is plan implementation. A Life Coach works closely, networking individuals and families with community churches and agencies, to implement the client's action plan. This includes life skill training, employment and /or public benefits, transportation and other issues that arise in meeting longer-term needs.
Help Finding Housing Resources: The final phase involves assisting individuals and families in moving into permanent, affordable housing. This includes landlord negotiation, homebuyer education, down payment/closing costs assistance through referral partnerships with supporting organizations.
